Housing Stabilization Services

Housing Stabilization Services (HSS) is a new benefit for individuals on Medical Assistance to help find and maintain housing.

Housing Stabilization Services has three parts:

  • Housing Consultation
  • Housing Transition
  • Housing Sustaining

Housing Consultation

Helps eligible individuals without a case manager or targeted case manager create a person centered plan and help submit the plan to a Housing Transition provider.

Housing Transition

We can provide support with all the details associated with moving, including but not limited to:

    • Developing a plan
    • Help with the housing application process
    • Organization
    • Researching housing options
    • Applying for housing
    • Ensuring adaptive accommodations are in-place
    • Coordinating details of a move (lease details, moving truck, supplies, etc.)

Housing Sustaining

The goal of this service is to support people to stay living where they want to! We are here to help with:

    • Education and training to develop good relationships with landlords, property managers, neighbors, and roommates
    • Support in understanding and applying for housing-related benefits
    • Transportation support
    • Assistive technology
    • Supporting arrangement of adaptive housing accommodations
    • Navigating Economic Assistance
    • Connecting individual with a Community Living Support (CLS) worker if transitioning from a group setting to an individual living arrangement.

Who is Eligible?

A member is eligible for housing stabilization services if they meet all of the following needs-based criteria:

  • Be on Medical Assistance (MA)
  • Be 18 years old or older
  • Proof of disability or disabling condition.
  • Proof of housing instability.
  • Proof of assessed need for services

How does the program work?

Housing Stabilization Services provides up to 150 hours per transitional for housing transition and up to 150 hour for housing sustaining per year.

What does a Housing Stabilization Coordinator do?

A Housing Stabilization Coordinator works with the individual to find them the appropriate housing choice, while coordinating all the other necessary activities required for a successful transition into the community.

We work to remove any housing barriers such as finding funding sources, housing record expungement, getting a replacement social security card, etc.
